To be responsible for conducting thorough investigations into serious incidents, critical incidents and complaints as necessary with the use of Root Cause Analysis methodology, ensuring Trust values of 'Being Open', ‘Empowering’ and ‘Compassionate’ are applied. Undertake assurance that patients, service users and their carers/families, Trust staff and where appropriate, third parties are fully involved in the investigation process and compile a comprehensive and detailed report to identify lessons learned and recommendations for development of action plans in conjunction with the appropriate manager. To undertake a debrief with management and staff of respective support services to ensure action plans are completed and appropriate learning takes place and is implemented. Show less
