•Completed 16 - week internship in acute inpatient and transitional polytrauma rehabilitation programs, working primarily with patients with diagnoses of traumatic brain injury, stroke, and amputations •Documented initial assessments, weekly progress notes and discharge notes •Created individualized treatment plans with measurable behavioral objectives •Provided verbal reports on patient progress during IDT meetings and family conferences •Facilitated individual and group sessions utilizing leisure education, cognitive based games, community planning, community reintegration, adaptive cycling, animal assisted therapy and aquatic therapy interventions •Co-treated with Occupational Therapy, Physical Therapy and Speech Therapy to address IDT goals •Internship Project: Developed standardized form for patients/families to complete after passes or outings to evaluate their progress towards treatment goals in their home or community setting, gain insight into their deficits, and identify areas for ongoing patient and family training and education.
