I was also responsible for instigating and carrying out an investigation into how DVLA assess medical fitness to drive (https://www.ombudsman.org.uk/publications/driven-despair). My proudest moment at PHSO came when I attended the Houses of Parliament to watch the Parliamentary Administration Select Committee hold DVLA and the Department for Transport to account because of the failings identified in the report.
