Clinical staff pharmacist | 25 bed Critical Access Hospital in Northern New Hampshire Daily activities include: interpreting & processing orders (inpatient, same day surgery, oncology), renal dosing, aminoglycoside/vancomycin dosing & monitoring, warfarin dosing & monitoring, therapeutic substitution, review of patient home medication list at admission, counseling & education at discharge (w/ follow up outpatient as needed), sterile compounding including hazardous drugs using Equashield CSTD, review oncology regimens & verify correct dosing and admixture, respond to codes and mix emergency medications, work with pharmacy director on policies and procedures, coordinate outpatient infusions (blood, antibiotics, IV biologics) - Created pharmacist home visit program - patients seen in their home to assist with medication management, understanding and adherence, in addition to preventing hospital readmissions. Visits range from a one-time consultation to weekly home visits indefinitely for high-risk patients who do not otherwise have a support system, with a range of services in between to meet unique patient needs. Close relationship with inpatient and outpatient case management teams to coordinate care. - Co-chair Antimicrobial Stewardship Committee - co-created Antimicrobial Stewardship Committee in 2017, developed and enforced policies related to antibiotic use (48 hour time-out policy, IV to PO policy). Review and present antibiotic prescribing trends within the hospital, review antibiogram, review and revise order sets related to infectious disease. - Integral member of EMR transition team throughout 2020. Assisted with system-wide switch to Meditech Expanse (from Mckesson Paragon) which brought together 3 hospitals, 4 rural health clinics and regional home health agency on a common EMR as of December 2020. Extensive formulary review and revision to work towards a common formulary among NCH health system. Show less
