Objective: To investigate the causes of two failure cases, fracture of piston rod at a radial oil port in clamshell excavator and failure at eye end in the backhoe excavator and provide a solution. • Analyzed the design of the hydraulic circuit mechanism in the excavator, found stress concentration in the design at the point of failure. • Studied the fracture surface of failed parts; found beach marks indicating fatigue stress. • Examined the list of failure cases; part has failed early compared to others. • Assessed onsite use of excavators; found very poor maintenance and abuse of machine by customers. Result: Root Cause Analysis proved the major causes to be poor maintenance and stress concentration. Recommended actions were servicing after 2000 hours of operation and increase of fillet radius. 30% savings in rework cost. Show less
