      I have recently completed the second half of my pre-registration training at a community pharmacy. These six months was split between working in a community pharmacy and working in primary care alongside a clinical pharmacist. My job role included dispensing, ordering stock, over the counter consultations, advising on minor ailments, helping to ensure the safe and effective running of the pharmacy, advising patients on the phone, referring patients to the appropriate services where needed amongst many other responsibilities. Working in primary care alongside the practice pharmacist allowed me to experience working in a multidisciplinary team setting and managing patients with both acute and chronic conditions. My role here consisted of taking part in medication reviews, following up on patients post-discharge from hospitals, amending records where necessary to ensure they achieve the best clinical outcomes possible. Alongside this, I was also able to conduct an audit and take part in medicine optimisation schemes to achieve clinical and CCG targets. This experience was marginally different from my time spent in the community pharmacy setting and the hospital environment due to the global coronavirus pandemic. With communication moving from physical consultations to online and phone communications I was able to participate a lot more in helping patients during this difficult time.

      JULY 2018 – JANUARY 2019 PRE-REGISTRATION PHARMACIST, THE YORK HOSPITAL As a Pre-registration pharmacist my roles during this placement were diverse and included rotations in the aseptic department , medicines information, clinical governance, dispensing and clinical trials. Patient facing roles included ward work on a range of wards including surgery, oncology, elderly and the acute medical unit. My primary roles consisted of taking medication histories from new inpatients and aiding in carrying out clinical medication reviews and ward rounds with medical practitioners. Alongside this I also had the opportunity to complete a clinical audit on the treatment of neutropenic sepsis. Other achievements of mine from this placement include hosting a antimicrobial awareness stall to help raise awareness of the growing resistance of antibiotics. My time at this placement allowed me to grow on my clinical knowledge and build my skills working both independently and as part of a multidisciplinary team.
