  Perform information and assistance tasks including completion of established intake procedures in order to arrange for needed services. Responsible for understanding and explaining services and programs that are offered by the Department of Aging to individuals contacting the Department seeking assistance or other information. Determine if the call is for information or a request for formal services. • Prepare and send appropriate information to consumers requesting services and/or information. • For referral calls, secure all appropriate information from the caller to complete the referral; determine if a Report of Need is necessary and forward the caller to appropriate Care Management staff.
