As a Crisis Case Manager my duties included, coordinated discharge plans, referring clients to additional outpatient resources and support services. In addition, I was involvved in assisting the transient population in obtaining housing and enrolling in placement services. As a Case Manager, I verify and/or arranged access to health insurance coverage. I acted as a patient advocate in regards to psycho education, community referral/assistance, family/support system contact, as well as education family members & involved significant others in mental illness and the treatment community. In my duties I am also a liaison and directly collaborate with lawyers, judges and public entities regarding inpatient psychiatric services and outpatient resources.
