      Full Circle Healthcare Solutions was founded with the intent of bridging the gaps in healthcare which affect patients when transitioning from one care setting to another and supporting patients once they have transitioned home. Full Circle Healthcare Solutions helps patients unravel the web of providers and medications with a focus on the patient’s recovery on their physical health and the impact behavioral health can have on the patients journey in life and health outcomes.We provide Patient Health Assessments and Rx Management to all our patients in support of the providers. Our PHARMCore Services are designed to discover a patients physical and behavioral health needs and the relationships they play on each other. These Services place the pharmacist at the center of the patients care and provides them with one unified source of communication, designed to help them adhere to the recommended care plan and provider ordered therapy. Our Transitional Care Pharmacist (TCP) reviews all aspects of the care being delivered to a patient and the support they are receiving. This review is designed to ensure there are no medication overlaps, adverse drug interactions, or adverse drug events. Working with our Behavioral Health team, our pharmacists researches why a patient may be struggling to maintain compliance with their current care plan. The TCP develops and recommends a new care plan to the patients’ primary care provider. This all-inclusive and supportive care plan is individualized to the behavioral and physical health needs of the patient. Our pharmacist becomes the patients partner in care, providing them education on their disease state and how to manage their medications. By providing for the patients physical and behavioral health needs, empowering them through education, and assisting them with access to resources, our care plans give patients the tools to adapt and cope with the challenges they face today, tomorrow, and beyond.
