I work side by side with clients with care coordination with providers, evaluations/assessments, placement assistance, psycho-educational counseling, treatment planning, housing assistance, family support, resource referrals and linkage, advocacy, assistance with daily living skills and money management, social skills, monitoring of symptoms, support, and individualized therapeutic interventions to assist in reaching their goals. With providing a wide range of services, our program strives to instill hope and empower those we serve to succeed in the community. We offer a wealth of available resources and tools allows those we serve to receive the support needed to achieve a productive life. Our services are focused on each client’s strengths, which we help them build on to achieve their goals.
