While on my intermediate practice placement I worked with 2 Occupational Therapists, alongside the multidisciplinary team to: - Assist with their caseload, and be responsible for a designated client caseload where I could decide priorities while balancing my placement demands. -Suggested implementation of a model of practice (Ecology of Human Performance) and completed a case study presentation to showcase this model in the current setting. - Undertaking initial and comprehensive assessments of clients, including those with diverse and complex presentations (for example hip fracture and "old" stroke) in their own homes, and documenting this on SystemOne - Using clinical reasoning to select appropriate interventions in conjunction with the B6 Occupational Therapists. - Completing a "TILE" risk assessment before assessment/intervention with clients, and discussing this with my educator and writing this on SystemOne - Liaising with and shadowing other members of the Joint Community Rehabilitation team including Physiotherapy and Podiatry. - Evaluation of client progress, including the use of the outcome measure TOM - Assisted and completed manual handling plans to be sent to care agencies and homes for clients. - Carry out and be responsible for home equipment provision, under the supervision of the B6 Occupational Therapist including moving and handling equipment.
