As the Senior Community Connector at Your Voice Counts I manage a team of Community Connectors across Gateshead and South Tyneside. We support people with learning disabilities and autistic people to identify and work towards outcomes they want to achieve, and to identify and address barriers to participation. We build links with people and services in the local communities so we can identify and develop opportunities and activities for the people we support. We work closely with Health, Social Care and Voluntary Sector services to ensure that people have access to the support they need and to ensure the voice of people we support is both heard and acted on.
