  I carry out Medicine's Management on the patients within Care of the Elderly, Medical, Surgical and Orthopedic & Womens Services.Within these wards, I carry out medicines reconciliations with the patients where I take drug histories from the patients, discuss their medication compliance and communicate with other members of the Multi Disciplinary team to optimise the medication for the patient and make adjustments where appropriate.I also then order medication where appropriate for the patient as well as ensuring that other factors such as care homes, community pharmacies and carers are aware of any changes and utilise items such as blister packs to maximise patient compliance.As well as these roles, I also carry out checking slots daily within the in and outpatient pharmacy and conduct patient counseling in the latter. I also am a team leader on the weekend and bank holiday roster when the pharmacy team is at minimum levels. For this, I produce rotas and organise all grades of staff to ensure that the wards receive an optimum service.
