They oversee and implement provision of the ECM services; and identification and achievement of Care Plan goals and objectives with the member that meet their self-identified strengths and health care and psychosocial needs. ⦿ Directly engage with high-needs MediCal members using health promotion and trauma informed care, and assess their needs to create individualized and longitudinal patient care plans. ⦿ Collaborate with interdisciplinary clinical and nonclinical staff across multiple facilities to coordinate care services for medical and non-medical patient needs. ⦿ Document patient information into internal electronic charting systems, and external health insurance provider portals. ⦿ Self-manage an appointment schedules across a 50-patient case-load.
