As a service coordinator, I provided concurrent case management services for approximately 40 families starting from program intake through age three, or until the delay condition was resolved. I coordinated evaluations to determine eligibility for therapy services, facilitated the scheduling of Individual Family Service Plan (IFSP) meetings and staffed therapists according to the child’s specific diagnosis or delay. For cases with ongoing service plans, I managed a multidisciplinary team of therapists to ensure that parent priorities, concerns and resources were being effectively met. I ran 6, 18 and 30 month IFSP meetings with families and therapists, and helped developed appropriate functional outcomes and objectives for therapy. I communicated regularly with all the members of a child’s therapy team including (but not limited to) speech, occupational, physical therapists and special educators. I reviewed quarterly progress reports submitted by therapists, as well as multidisciplinary evaluations. Additionally, I attended relevant training seminars on topics ranging from Autism, motor disorders and speech/language disorders.
