  Under the supervision of the Center Manager, this position is responsible for the delivery of clinical services and community supports to persons with severe and persistent mental illness. A crisis case manager(CCM) must provide after hours on-call services. CCM must also coordinate with all external providers and referral sources to ensure appropriate referrals are admitted to the program, and facilitate the coordination of the resident intake/discharge and the referral process.
