Some of my responsibilities include tracking and reporting on long stay patients, coordinating meetings for those with complex familial, immigrant, or legal situations, and coordinating with inpatient social workers, case managers, and hospital leadership to align all parties and move discharge plans forward. As an ambassador of the Central Flow Unit, I participate in staff meetings and initiatives including a pilot program for inpatient flow medical directors, identifying hospital wide system-level issues, and standardizing transportation across all departments.

As the liaison, I help to coordinate referrals for select homeless patients that have been referred to BMH and relay information between the patient's physician and care team and BMH regarding potential discharges. I assist in identifying and triaging medically and socially appropriate patients for admission consideration at BMH and flag high utilizers to discuss during monthly meetings with the two organizations. Other responsibilities include collecting data on high-risk homeless patients to assist with process improvement initiatives, identifying weaknesses in operations and recommending areas for refinement, and communicating with Boston shelters as necessary.
