Trenton Health Team is an innovative multi-sector partnership focused on improving the health and well-being of New Jersey’s Capital City. THT was founded by Trenton’s largest healthcare entities, including St. Francis Medical Center, Capital Health, Henry J. Austin Health Center, the City of Trenton. Since inception in 2006, the partnership has received national recognition and grown to include more than 60 community organizations that play a critical role in improving the health of the community.THT’s strategies include:- Serving as the “backbone” for health improvement and convening diverse partners in Trenton around health, wellbeing, and health equity. These efforts include active partnerships on the local, regional, and national levels, to align efforts and share insights on best practices- Providing direct interventions for community members in need, including pilot programs, especially focused on priority populations- Utilizing data and data analytics as a tool to improve health and wellbeing. This work includes operating a robust regional health information exchange that includes real-time population health analytics and incorporates data from clinical, claims, and social determinant datasets- Advocating for policies that impact health and wellbeing and helping our community partners grow their capacity to support policy changes as well.Major projects:- ReThink Health Ventures - 1 of 6 partnerships nationally- BUILD Health Challenge - 1 of 17 coalitions nationally- 2015 Robert Wood Johnson Culture of Health Prize Finalist - 1 of 16 selected nationally (and only one in NJ) from more than 200 applicants- Certified as 1 of 3 Medicaid ACOs under NJ’s Demonstration Project (57,000 lives)- Trinity Health - Transforming Communities Initiative community (1 of 8 nationally)- National Quality Forum: Population Health Action Guide Field Testing Group- Featured in Agency for Healthcare Research and Quality’s Innovations Exchange

The Trenton Health Team is a community health improvement collaborative serving Trenton, N.J. The collaborative is an innovative partnership among St. Francis Medical Center, Capital Health, Henry J. Austin Federally Qualified Health Center and the Department of Health and Human Services of the City of Trenton. The vision of the Trenton Health Team is to make Trenton the healthiest city in the state. Our mission is to transform healthcare for the city by forming a committed partnership with the community to expand access to high quality, coordinated healthcare, while being ever mindful of the necessity of containing costs that have spiraled out of control and are now unsustainable.Major initiatives include:- Community-wide Clinical Care Coordination Team (C4T) and Care Management Team identifying gaps in care, addressing social determinants of health, and providing wrap-around services to the highest-utilizers of Emergency Department services- Development and Implementation of the Trenton Health Information Exchange- Substance-use/misuse screening and treatment through the grant-funded SBIRT (Screening, Brief Intervention, and Referral to Treatment) program- Unified Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP)- Restoration of pediatric, adult, and specialty care services in the City of Trenton Free Clinic- Formation of a Medicaid ACO under the NJ Medicaid ACO Demonstration Project
