  • Highly motivated and passionate occupational therapist at a skilled nursing facility with four patient units including post-acute rehabilitation, long-term care, neurobehavioral rehabilitation and Alzheimer’s/memory care. • Perform evaluations and create patient-centered interventions for both short and long-term clients based on a variety of functional deficits. Treatment includes ADLs, IADls, patient, family and staff training, splinting, pain management, functional transfers, adaptive equipment for feeding, wound care, and wheelchair positioning. • Utilize Accelerated Care Plus modalities such as electrical stimulation, short wave diathermy, and ultrasound for pain management, range of motion, muscle re-education, wound healing, and strength to maximize functional performance and quality of life. • Participate in frequent interdisciplinary meetings, collaborating with patients, family and staff throughout a patient’s admission. • Oversee the implementation of treatment plans through two full-time Certified Occupational Therapy Assistants, working together to maximize patient outcomes. • Perform in-home assessments to provide custom recommendations for patient safety to ensure a successful transition home. • Conduct facility audits to measure the follow through of therapy recommendations, such as splints and adaptive equipment, which increase patient quality of life and reduce department/facility liability. • Facility Dementia Capable Care Champion- assess patient’s cognitive performance via the Allen’s Cognitive Levels and utilize the information to help increase patient’s functional participation in ADL’s and other meaningful tasks.
