      •Outreach to referred homeless older adults from various agencies to assess for mental illness needs, housing needs, and health needs to determine eligibility for Full Partnership Program (FSP) •Conduct strengths based, culturally competent individualized assessments for older adults by gathering information including healthcare, substance abuse, co-occurring disorders, history of mental illness, employment, or housing from member, family, and/or involved agencies as appropriate•Provide rehabilitative services to members and/or families which includes assistance in restoring or maintaining a member’s functional skills, daily living skills, social skills, and/or linking up with needed community services and support resources•Work with treatment team (case manager, nurse, psychiatrist, peer advocate) to provide appropriate coordinated care services in the community for the member •Co-facilitate a biweekly caregiver support group for members that are caring for an aging loved one by providing psycho education about illness, linkage to resources in community, individual therapy as assessed •Complete DMH progress notes and DMH required documentation within two to three business days of service delivery
