  · Proficient in all aspects of filing for Social Security Disability (SSD) and Supplemental Security Income (SSI) for Hopewell residents · Obtain all current and past psychiatric records on current residents via hospitals/outpatient mental health facilities throughout the US and imputing data into online application/report on SS website · High success rate (98%) in obtaining SSD/SSI benefits for residents · Upon completion of SSD/SSI application, work in conjunction with assigned adjudicator(s) with Opportunities for Ohioans with Disabilities Division of Disability Determination including the completion of MSQ and DAQ · File appeals with Social Security for residents if SSD/SSI denied · Apply for Ohio Medicaid and/or transfer Medicaid from other Ohio Counties to Trumbull County · Research and coordinate transition planning for residents when discharge approaches, including housing options and follow up mental health care and appointments locally and nationally · Accompany residents on local housing options prior to discharge · Enter admission data in Hopewell database when resident is admitted · Assist with intake/admission duties when needed · Work with nursing to get an override on private insurance for prescriptions Show less
