I provided decision support to older adults and adults living with a disability to determine which resources or services best fit their long term care needs. I conducted needs assessments and explained the availability and limitations of resources so that individuals and families understood what their options were for staying as independent as possible as they aged. I assisted with program development that continued to improve the documentation and reporting requirements and establish the boundaries of the Options Counseling program. I provided presentations to service providers, community members and stakeholders explaining the value of the services provided by our agency.

Care Transitions Intervention (CTI) Coach
