AFEA Care services provides aged & personal care services to NDIS approved clients. As a care coordinator my job is to ensure that I review client’s care plan and screen the care givers for their expertise and assign them to provide the caring services. I take full ownership of developing services schedule for the client and ensure no service is skipped by the care givers. Coordinate services as defined in the care plan to include mainstream NDIS services (housing, transport, health) and specialist services. Regulate the usage of support hours and update the client if additional care plan assessment may be required. Use an in-house client relationship management (CRM) software to record discussions with clients, details & modifications to their services, carers used. Use CRM effectively to monitor the funding and services are booked adequately. Provide reports reflecting progress towards achieving plan goals, and recommendations about the appropriate type of support required in the next plan and reasons for this. Development of new services as required. Positively contribute to the team by actively participating in team meetings, attend training as required, provide outside office hours support to clients for a week in a month.Dealing with clients in a courteous demeanor over phone & email, liaising with external agencies to prepare services & book them accordingly. Comfortable using Microsoft suite of applications such as Word, Outlook, Excel, PowerPoint and SharePoint.
