• Obtain diagnostic reports and confirmation of disability by appropriate medical or mental health professional and assist with follow-up. Discusses diagnosis with parents and obtain consent for services. • Coordinate with LEAs in developing an Individual Education Plan (IEP) for each child with a disability. Attend and facilitate parent conferences in collaboration with site staff. • Assures accurate, complete, and confidential maintenance of all children records, and the forms are up-to-date and comprehensive. • Regularly consult with parents and staff on the progress of the disability/mental health services and the individual children enrolled in the program. • Arrange for the development and implementation of services for children with disabilities or suspected disabilities. • Arrange classroom mental health observations and follow-up. • Arrange, coordinate, and monitor all services to children with a disability, including contractual and CST services. • Actively engages parents and Mental Health Consultants in planning and implementing the individual mental health needs of children and families. • Observe children and consult with site staff and Mental Health providers to develop appropriate intervention strategies, as needed. • Advises and assists in developmental screenings and assessments and takes appropriate actions regarding findings. • Collaboratively arrange regular mental health group meetings for parents and program staff in cooperation with the Mental Health/ Disability director, Mental Health Consultant, and Mental Health agencies. • Implement the Mental Health/ Disability’s Referral Process.
