As of January 1, 2018, Lifetime Health Medical Group has permanently closed operations. The Amherst, Hamburg and Mosher Health Centers are permanently closed. The Greece, Perinton and Wilson Health Centers have transitioned to Rochester Regional Health. The Artemis Health, Westfall Pediatrics and Folsom practices have transitioned to UR Medicine.
