Our services aim to improve the quality of life of the foreign population living in Berlin by offering personalised and targeted assistance to improve the quality of health services offered on German territory. We want to develop territorial care and, where necessary, find alternatives to meet the new health needs of foreigners on German territory. Empowerment, self-care, primary care, case management and Chronic care model are our care models.
