At Hope, we help transform the experiences of illness, aging, and dying into opportunities to live well. Many of our programs enable adults to live comfortably through their later years and into their final chapter of life. We also give hope to children coping with complex medical issues, serious illness, and grief. Isolated seniors can look forward to warm meals delivered to their home, caregivers who deliver medicine and monitor their health, and aides who provide housekeeping. During serious illness, participants will receive help navigating the complexities of their treatment. We connect families with useful resources and provide comprehensive education for caregivers. Detailed care plans are customized to meet each person’s needs and may include varied services such as medical and dental care, therapies, home and personal care, medications, medical supplies, and essential transportation. For isolated or lonely seniors, warm meals and social activities at Hope Care Centers can be a welcome change. By staying within Hope’s continuum of care, these individuals can receive uninterrupted services, while avoiding unnecessary hospitalizations or emergency room visits. With our support, patients, families, and caregivers will find the help they need for the social, emotional, and practical challenges that may result from serious illness. This comprehensive model of care has garnered the attention of health care agencies, policy makers, and associations around the country.
