Our solutions are tailored to meet each clients’ needs based on the type of difficulty they are experiencing. These service difficulties – or dilemmas – arise when consumer needs transcend existing services which are traditionally provided through disease management, case management, care coordination and triage models of care. HealthNav™ services are provided on a fee-for-service basis.
