Overview

The Health Alliance was established in 2017 as a partnership between Metro North Health and Brisbane North Primary Health Network (Brisbane North PHN). The Health Alliance has been driven by an impetus to think differently about how to achieve optimal and equitable health outcomes for people in the North Brisbane community. The Alliance has also been described as “Switzerland”, as a “place where parts of the sector can meet as equals”. It was created from a recognition that: - complex health and healthcare problems that transcend the mandate of any one organisation cannot be fixed by existing approaches. - Australia’s current funding and operational models of health care do not always deliver the best outcomes for the population, particularly those who are vulnerable or of greatest need. Our current priorities include: 1. developing and embedding initiatives designed to enable care to be provided closer to or in a person’s home. 2. improving health outcomes for children born in the Caboolture region. 3. mental health co-commissioning planning 4. complex care models to reduce ED demand at Caboolture Hospital We achieve our outcomes by: - facilitating a neutral space for stakeholders across the interface of hospital and primary care, to meet together on equal ground and jointly generate integrated solutions. - understanding the health care needs of our community through analysis and planning, helping to identify and address service gaps. - driving person-centered and capacity-building health care models, that empower communities in the pursuit of the best possible health outcomes.
