At First Coast Aging in Place, we value your goals to remain safe and independent in your home. We help identify, prioritize, and address potential safety issues in your current environment, and challenges that may arise in the future to help you maintain peace of mind to age in the location of your choice. We pride ourselves on developing therapeutic partnerships that support your goals by providing several clinical services to support aging in place successfully. As advocates for community-based services and aging-in-place, we provide the tools, resources, and education to help individuals safely and successfully return home instead of alternative placement options. We provide tangible and easy to integrate tools and techniques that can help reduce fall risk, increase independence, and help patients and families have peace of mind with the transition home after a hospitalization, surgery, or other medical procedures. Our clinical education, experience, and training, we offer the following comprehensive services:· Direct Occupational Therapy Services: -Skilled rehabilitation services to increase strength, endurance, mobility, and safety to increase participation in Activities of Daily Living (ADL’s) -Education and training on compensatory techniques, use of adaptive - equipment, and use of durable medical equipment to increase safety and independence· Home Safety Modification Services: -Comprehensive assessment of an individual’s current abilities, prognosis, and future disease process to plan for future home modification needs -Assessment of physical environment including suggestions to remove environmental barriers and set up environment to decrease fall risk and and increase safety· Memory Care Services using evidence-based Skills2Care program: -Evidence-based tools and techniques to manage challenging situations -Specialized caregiver training to reduce stress and decrease caregiver burden while managing caring for someone with Dementia
