Overview

Product: Sigma Digital Records (SDR) is a documentation software created for therapists in order to have a more accessible and efficient digital file on record. It can be stored in a hard drive or in a cloud for easy access. It eliminates the inconvenience of hand-writing and space-occupying storage of printed files in steel cabinets. SDR is laid out to make searching of patient names, record numbers, and dates very easy and efficient instead of going through each printed file in storage boxes and cabinets one by one. Sigma Digital Records (SDR) is designed to contain different levels of access and clearance including HR level, director level, and therapist level. The software is currently developed in Windows platform but will be expanding to include iOS and Android applications. Sigma Digital Records (SDR) documentation workflow include clocking in/out of therapists, assignment of patient to different disciplines, and scheduling and time management of patients and therapists. Notes done by therapists when using SDR include: - Evaluation - Daily notes and billing - Progress notes - Recertification - Discharge notes Sigma Digital Records (SDR) incorporates use of ICD 10 codes and CPT codes for diagnosis and treatment respectively. History and Physical Examination tabs are organized to contain relevant information including: - Chief complaints and patient medical history - General ROM and MMT assessments - Specific ROM, MMT, and pain assessments - Balance, Bed mobility assessment, Transfer assessment, Gait assessment, orthosis/prosthesis assessment, and assistive device assessment - Short term and Long term goals - Standardized tests and other related assessments Currently, the application of Sigma Digital Records (SDR) is documentation for use by rehabilitative professionals with aims to increase work productivity while keeping an easier and efficient note taking. SDR will expand its scope in the future to include documentation for use by Physicians.
