Receive referrals from the Elderly Affairs Division to provide case management for senior who are 60 or older. Conducted intake, assessment, and support plan. Then implemented support plan by linking to referrals and resources. Monitor case monthly. Visit clients quarterly at their homes to review support plan and make adjustments. Every 6 months reassess clients at their home. Works collaboratively with an Aide who will assist with all cases.
