Huron Valley PACE®, or Program of All-inclusive Care for the Elderly, located in Ypsilanti, offers an alternative to nursing home care for participants, providing comprehensive health care – all in one place. With a holistic approach to your overall health needs, Huron Valley PACE offers a one-stop shop that embraces you like family. We’ll even pick you up and bring you back home! To join Huron Valley PACE an individual must: Be age 55 or older. Be certified as meeting the State of Michigan criteria for nursing facility level-of- care. Live within the Huron Valley PACE service area. (including Washtenaw and Monroe counties, as well as part of Oakland, Wayne, and Livingston counties). Be able to live safely in the community with the support of Huron Valley PACE. **Nursing home level of care means someone who is struggling physically or with their memory. We evaluate with a simple phone call followed by an eligibility assessment provided by our PACE intake team. Paying for PACE If a person meets the income and asset limits to qualify for Mediciad, this insurance pays for a portion of the monthly PACE premium. Medicare pays for the rest. If a person does not qualify for Medicaid, he or she is responsible for the portion of the monthly premium Medicaid would pay. Participants begin services on the first of the month and enrollment contiunes as long as desired by the PACE participant., regardless of change in health status. A participant may be fully liable for the cost of medical services from an out-of-network provider or without prior authorization (except in emergency services).
